By booking this appointment I am agreeing to having either a face to face or online assessment/treatment/class by Body Works Physiotherapy and Pilates.

If attending via an insurance scheme, I am responsible for checking that I am covered for my treatment and agree to pay any excess payment charge (if applicable) or any outstanding charges not paid by my insurer.

We request that if you are a new client of Body Works or have not been seen by us in the last 6 months, you complete the online registration form which will be sent to you before your appointment.

I am happy to receive clinic updates/appointment confirmations and exercise prescription via email.

I consent to (where appropriate) update reports being sent to my Gp/consultant/solicitor/insurer.

Please note, in order for clients to attend our clinic in person it is essential that you read and follow the guidelines below:

– Only come into the clinic if you do not have any signs or symptoms of COVID-19 infection (raised temperature, recent persistent cough, recent loss of your sense of smell/taste).
– Avoid the clinic if you’ve been in contact with anyone, in the previous 14 days, with confirmed or suspected COVID-19 infection.
– Wash your hands on entering and exiting the clinic (there are basins in all treatment rooms, as well as in the two bathrooms).
– Provide your own face mask (or buy one from the clinic) to use when treatment compromises the 2 metre distancing guidelines.
– Fill out any screening forms online prior to coming into the clinic – to eliminate the need for use of paper/pens and writing boards within the clinic.
– Turn up for your appointment at the allocated time. Do not arrive early. This will allow you to come straight into the treatment room instead of waiting in the reception area.
– Restrict the items you bring into the clinic. We request that you avoid bringing any unnecessary items into the clinic (eg. bags, coats).
– Where possible, come into the clinic by yourself. We appreciate that some of our clients do require assistance to get to the clinic and we welcome you. Also, we emphasize that clients under the age of 18 do still require a guardian to be present throughout their appointments.
– Please pay for your treatment (before your session) by bank transfer or via our online portal. But if you are not comfortable with this our usual payment methods are still available.
